What We're Doing:

• This year, we will review each Emergency Department visit to better understand reasons for transfer and how we can continue to support care within the Home
• We will bring our care teams together regularly to look at trends, learn from these experiences, and make improvements to support safe, timely care

Our Equity, Diversity and Inclusion (EDI) Quality Improvement Plan focuses on creating a more welcoming, respectful, and culturally safe environment for everyone we serve and work with. This year, we will continue to build knowledge and awareness by supporting leaders to complete Indigenous Cultural Safety training and expanding EDI and anti-racism education to front-line staff through the first module of a new learning series. By making this education accessible and encouraging participation, we aim to strengthen understanding, reduce barriers, and support more inclusive, person-centered care experiences for clients, residents, families, and staff.

Current Performance: The percentage of residents taking a regular antipsychotic medication is 26%, with steady improvement over the past year.

Our Goal: Reduce the percent of residents on these medications to 20%.

What We're Doing:

• This year, we will strengthen regular medication reviews to ensure medications are still needed, used safely, and adjusted when appropriate to best support each resident's well-being
• We will begin expanding the Living and Working Well education series for staff to support more non-medication approaches in everyday care

Current Performance: The rate of worsened stage 2 to 4 pressure ulcers remains above the provincial average.

Our Goal: To decrease the percentage from 3.74% to 3.30% to reach the provincial average.

What We Are Doing:

• This year, we will ensure residents at higher risk have personalized repositioning plans in place to support comfort, skin health, and prevention
• We will bring our care team together weekly to review any new pressure injuries, understand what may have contributed, and take action to support healing and prevent further issues
